I confirm that my organisation qualifies as a small and medium sized enterprise as defined in the EU Recommendation 2003/361/EC

We represent an EU member state company according to the EU definition (Commission Recommendation 2003/361/EC). The fulfilment of this request is made with an official declaration issued by the authorities of the SME headquarters address (c.f. Attachments).

We confirm that our company is legally incorporated and established in one of the EU member states. The ownership or shares are not held by individuals or entities outside the EU; ownership of shares are solely held by legal persons that are incorporated within the EU. The complete fulfilment of this request is made with an official declaration issued by the authorities of the companys headquarters address, together with information about the ultimate beneficiaries of the companys shareholding, when applicable.

We have the resources to cover the expenses not covered by the TEF Health discount on the selected services.

We confirm that we have not received more than 300,000 EUR in state aid support during the previous 3 fiscal years (EU Regulation nº1407/2013 of the European Commission). The fulfilment of this request is made with an official declaration issued by the authorities of the SME headquarters address (c.f. Attachments).
